Port forwarding

I am running a dedicated server, but cannot see it on the master server. Suspect the problem is with my actiontec 1594 router /modem (Qwest ISP). Tried to port forward 28960 thru 28964, but don't know which IP address to forward to/from (not a network guru). When logging into the router I have 3 boxes to input data, where two are for the start /stop port numbers, ie 28960 to 28964 in this case and a ip address. Don't know if the IP address to be inserted is on my side of the router or the master server Ip. Any help will be appreciated.

The port should be 28960 and the IP should be your server's local IP address. Probably something like 192.168.0.2
