The easiest way to make a config (for me) is to use a config maker, and add all the other stuff you need in it after it's done. I think you can find a program here in the downloads section, but you can also find online ones that do the same thing by simply searching google...

You'll need a mod to disable pistols and grenades (or at least make it so you get no ammo for them), there are no cvars for them

Here's (http://www.cod.jolt.co.uk/index.php?page=download.php&action=filenotes&id=2027) a mod to disable ammo for pistols. It's easy enough to modify the grenade scripts so players don't start with any.
